~ Club Officer Form fillable form
It is very important that your CLUB PROFILE is completed for the new membership year and returned to the MTFRW president ASAP following your Club elections.
Please review who should have access to the NFRW database for your Club. This person, or persons, can update your CLUB website at the National level. For example, posting campaign volunteer hours for your Club. Individual hours must be logged by each member themselves. To fill out the form, tab from field to field (or click in each field) and fill in your officers information; Download (arrow in upper right corner) and save to your files with your Club’s name in the filename and send that file as an attachment to the MTFRW President.

To fill out writeable, or fillable forms, tab from field to field (or click in each field) and fill in your information; Download (arrow in upper right corner) and save to your files with your with a specific filename and send that file as an attachment to the MTFRW President.
Note: you cannot forward a fillable form, it must be downloaded and then saved with a name-specific filename. If you do not save the file, the form reverts back to empty fields and your information is lost and needs to be reentered, then saved.
